SIGN & RETURN:

Parent-Athlete Agreement Information – Digitally sign below, no later than 3 days after Parent Meeting

Concussion Information Form – Digitally sign below, no later than 3 days after Parent Meeting

Emergency Medical Form & Bus Safety Rules – Print, sign and return to coach the first day of practice

Lindsay’s Law (Sudden Cardiac Arrest) – Print, sign and return to coach no later than 3 days after the Parent Meeting

Pay to Participate – Return to Payment Depository outside the main office by due date
